Scheduling an Appointment

Scheduling an appointment is easy!

Some of our sites offer extended hours for early morning and evening appointments and some have weekend hours.

To schedule an appointment, ask to speak to a Customer Service Representative at the location nearest you.

When you call, a Customer Service Representative will ask for basic information to locate your record in our system — or will work with you to set up an initial intake appointment. Please have birthdate and insurance information available when you call.

If you need service in Spanish, just let us know. Some of our Customer Service Representatives are bilingual and speak both English and Spanish fluently.

To schedule an appointment with one of our providers, please contact your local office directly at the phone number listed for the location nearest you.

Rescheduling or Canceling an Appointment

If you are more than 10 minutes late for your appointment, we will try to work you in as availability permits. If you are late and there is no way to work you into the schedule, you may be asked to reschedule.

Please, always let us know at least 24 hours in advance should you need to cancel an appointment so we may accommodate someone else who is in need of an appointment.
