Our staff of highly skilled health care providers deliver quality care across a wide range of services.
Step 3 – Schedule your Appointment
Once you’ve located your health center and chosen a health care service, contact us at the number listed at the location nearest you to schedule an appointment. Most of our centers offer extended hours for early morning and evening appointments. Some centers also have weekend appointments available.
Our Customer Service Representative will ask you basic questions to set up your record including name, address, date of birth and your insurance plan.
