Training Requests

Horizon Health and Wellness, Inc. recognizes the importance of training and developing employees. Horizon encourages staff to be continually improving their skills and abilities through internal and external training and education.  The training department is always here to assist with your training requests in an efficient and timely manner.  Here are a few tips to make the process as seamless as possible.  Please keep in mind that recording is a critical part of compliance. 

Documentation

All training reimbursement requests are to be sent to training@hhwaz.org for processing and recording. The training request form has been updated and can be found on the company intranet, in the Forms section.

Submit all documentation (i.e., receipts, proof of attendance, etc.) directly to the training department after training is completed. 

  • Be sure all approval signatures are in place.
  • Training will record and then forward to accounts payable for reimbursement when applicable.
  • If using personal credit card payments, receipt(s) are required as proof of attendance.

For leaders who have P-cards and choose to pay for your own or your employees’ training expenses in advance, you may do so, however, all of this needs to be sent to training@hhwaz.org for recording.

 The training department will provide documentation to Accounts Payable.  Documentation submitted must include proof of attendance/completion of training.

Any training requests (i.e., conferences, seminars, on-line training, etc.), that require payment in advance, please enter that on the tuition line of the form in addition to the tuition amount of the training. 

If you elect for the training department to register you for the training, you must include the appropriate website in the description box on the form, so that we will be able to register and pay for the training.

The training department will notify staff and the supervisor will be copied once registration and payment is completed.

If after receiving a request(s) and more information (i.e., receipts, signatures, etc.) is needed, the training department will reach out to the requestor.
